Try Phil-a-form for surveys. I built several of them using it. It's scalable and it saves the records in the database. It's commercial but very cheap

I think that the easiest solution for you would be to use a Paypal button on a content page. There are several solutions, but I like to use the RAWCONTENT component. The site looks professionally done, but is the "picture of the day" so important to go above the fold? I would move it to a lesser spot and place news and static text there instead.

Otherwise, I find the site's color scheme unusual and pleasant at the same time.
