Customised surgeon search (looks to be a variant on Peoplebook)
Customised login / registration sequence.
Apart from that, it seems to be the standard J! install. (and no, I wasn't involved in it, though I do know the various developers through multiple FOAF/XFN)..
Ideally, they need to deploy some form of SEF solution (OpenSEF RC5 or RC6 when it comes out).
