How do I add fields to the contact us page?

I have a customer who would like to use the contact us page as an order form... and have an address field and information field for what will be printed on his product. Should I do it this way or is there an extension/module that I could add to do this job better?

You would be much better off looking at one of the Contact type extensions on the extensions directory.  I would be surprised if there wasn't something else out there that could accomplish that same task already for Joomla!.
